This is a widely debated subject as there are no clear lines between them.
It also depends on the area of practice.  In my opinion:

Designer:  Designer is someone who can put together things.  For example in
a steel building, this person would be able to layout beams, columns,
foundation.  Junior engineers are sometimes called designer. In the
architectural world, normally young architects start as designers and then
progress to the level of Junior architect etc.  The younger generation
engineers are getting trained so that the junior engineer now can do the
design and the drafting.

Detailer:  This is mostly used in the steel shop drawing creation where
extensive detailing is required.

Drafter:  A person who drafts - draws things that are given to him - he is
not required to think much about the framing.  So nowadays, people don't
like to called drafters but designers.

Hi,
In structural Engineering:

DESIGNERS: One who visualises the behaviour of the structures and arranges for appropriate members for the structure to perform its function.
DETAILERS:One who thinks of the detailing of the members so that the idea of designer can be implemented in practice.
DRAFTERS:One who prepares the drawing and incorporates the idea of the designer and the detailer.

They all can be the same or different persons.
